Does filler migrate? The filler moustache, and how it is fixed.

Filler does not wander by itself; it is placed wrong or in excess. Injecting above the lip border, or syringe upon syringe, makes the puffy ledge. The fix: dissolve, wait, redo with less.

Lip filler · 3 min · Dr. Sara El Hamawy · October 2026

"My filler moved" is a sentence we hear often from patients arriving from other clinics. In truth hyaluronic acid filler does not travel on its own. One of three things happened.

1. It was placed in the wrong spot from the start

The lip has anatomical borders. Superficial injection above the vermilion border puffs the skin above the lip, which reads as a "filler moustache" or duck lip. The product did not move; it was put there.

2. More than the lip can hold

A lip holds a certain volume. The excess is pushed upward and outward by talking and eating, especially after heavy pressure in one session or a syringe every few months before the old one has gone.

3. A firm filler in a delicate place

Cheek filler placed in lips shows as lumps and pushes outward. Lips need a soft product made for them.

The fix

Hyaluronidase dissolves the wrong part within a day or two. We wait two weeks for the lip to return to its true shape, then redo with less, in the right layer. Most cases need only half a syringe after a correction, not a full one.

Quick answers

Does dissolving ruin the lip?

No. Hyaluronidase breaks down the filler only; your own hyaluronic acid regenerates within days. The lip returns to how it was before filler.

Can I dissolve and refill on the same day?

Dr. Sara prefers not to. Two weeks between the two makes the assessment honest.

How do I avoid it in the first place?

One syringe per visit, a soft lip-specific product, a doctor who injects in the right layer, and no top-up before the old one has faded.
